Initial Visit – Evaluation & Treatment
We are very happy that you have selected RESULTS PT for your physical therapy services.
Attached are the patient registration forms to be filled out completely and brought with you.
I accept cash or check at the time of service. Your services may be covered by your medical insurance. You must call your insurance provider to check your deductible, co-pay amount & physical therapy coverage. RESULTS PT will provide you with a paid invoice for each visit. If you choose, we can forward your invoice to a local medical biller that will process your insurance claim with the reimbursement check coming directly to you from your insurance provider.
Please call me directly if you need to change your scheduled appointment. Your initial visit will take approximately 90 minutes.

- YOU NEED TO DRINK 2 BOTTLES OF WATER WITHIN 2 HOURS PRIOR TO YOUR TREATMENT SESSION.
- PLEASE BRING ATHLETIC SHORTS AND LOOSE FITTING TOP / SPORTS BRA FOR TREATMENT SESSION.
- BRING A WRITTEN LIST OF YOUR MEDICATIONS
Bring with you any doctor medical reports & studies, MRI, CT and X-ray imaging with the written reports from the radiologist. Call with questions.
